Practical Nurse (LPN/LPT/LVN) provides direct and indirect care to
clients in their home environment in accordance with the
organizations policies and procedures. The LPN instructs,
motivates, and assists clients in health maintenance and disease
prevention to improve their ability to live independently at home.
The nurse directs the efforts of the Caregiver when involved in the
clients plan of care. Based on state regulations, may perform
administrative and/or field/clinical responsibilities while
receiving oversight and clinical direction from a Registered
Nurse.External Job Description:Provides skilled nursing
care/treatment to clients per Physicians orders and Plan of
careAccurately documents nursing actions of all care given,
individuals health status, and communicate with family and
physiciansDocuments the progress and outcomes for established goals
and informs physician, case manager, and families of changes in
client's medical condition and needsProvides skilled nursing
treatments per physicians orders and within the scope of
practiceMedication administration and management as neededProvides
health teaching of client and assisting client in learning
appropriate self-care techniques and safety measuresConducts
nursing assessment visit as neededReports all incidents per
operation procedure and regulatory standardsFollows policies and
procedures according to operation, state and regulatory
standardsUses safe body mechanics to prevent personal injury to
self or clientMaintains confidentiality of patient related
information and honor patient rightsConsults, seeks advice, and
follows direction of the RN in fulfillment of care of the
clientCompletes Start of Care assessments in person, where
applicable and in accordance with state regulations (as
applicable)Completes supervisory visits based on compliance and
regulations to include review of plan of care, caregiver competency
assessment, and completion of required documentation. Completes
daily, monthly, quarterly and annual nursing documents as/if
required.Works closely with Director or Nursing/Nurse Supervisor
and/or Branch Manager in planning, implementing, and evaluating
programs and services to address the health needs of persons
servedAttends and participates in staff meeting, in services, and
other meetings as requestedEnsures compliance with
state/local/payer requirements and that documentation reflects the
care provided and meets company, regulatory, and payer
standardsEnsures the ordering, reviewing, monitoring,
discontinuation and, if necessary, the disposal of medications as
prescribed by the physicianTranscribes or ensures transcription of
physician phone orders and other medically related reportsEnsures
that furnishings, equipment, and supplies are maintained within
guidelines established to prevent the spread of diseaseEnsures that
appointments for health related issues are scheduled, completed and
that appropriate response to findings are implemented in a timely
mannerCommunicates with Branch Manager, Customer Service Manager
and/or Scheduler when clients are hospitalizedCommunicates
effectively and routinely with supervisor, ensuring that pertinent
information reaches supervisor in timely mannerProvides new
employee training, annual re-certification training, and competency
validation for health and safety related topicsMay be asked to
cover hours, weekends, and holiday on call to meet the needs of
clientsOther duties as assignedQualifications:Degree in Nursing
from an accredited schoolExperience in administering medicationsOne
or more years of nursing experience preferredHome health care
experience preferredCurrent and unrestricted license in the state
of the operationCPR professional certificationBasic clinical
skillsProfessional appearance and demeanorGeneral knowledge of
computers and the ability to learn industry specific software and
documentation applicationsExcellent oral, written, and
interpersonal communication skills with all levels of personnel,
internal and external to the companyExcellent organizational and
time management skills with a keen attention to detailsCapable of
working responsibly with highly confidential informationAbility to
work effectively independently or in a team environmentAbility to
develop critical thinking skills, research situations, solve
complex problems and deal with a variety of issuesAbility to learn
