Job Description:

The Case Manager provides direct casework management, counseling, or mental health services to children and families within the agency's policies and program context, utilizing human service principals in developing relationships, assessment, and development of service plans.Job DutiesArranges for and provides transportation for youth, such as school appointments, medical appointments, court and ACRsRefers and links youth to necessary resources to meet needs, including external therapy, medical and educational resources, and psychological and psychiatric specialistsArranges visits with youth with families, including siblings and visiting resourcesAssists with the completion of Mental Health Assessments, Individual Treatment Plans, Individual Treatment Plan Reviews, and Clinical Summaries; completes Discharge SummariesParticipates in individual treatment plan meetings, reviews, additional staffings, transition staffings, and child and family team meetingsParticipates in court proceedings on behalf of the youth; this includes providing testimony at hearings, compiling court reports, and preparing case recordsCompletes and participates in Administrative Case ReviewsAssists youth in determining transition and discharge plansActs as a liaison with multiple public and private agencies to advocate and assist with service needs of the youthContinually trains and maintains certification in the implementation of Therapeutic Crisis Intervention (TCI) or other approved behavior modification programFollows OHU Residential Program Plan, including daily schedule, administration of consequences, and mental health treatment of youth in residential careDemonstrates the highest investment in and commitment to quality improvement practices in order to continuously improve the delivery of services to our residentsParticipates in a weekly Clinical Team Meeting of assigned cottage; moderates staff team meeting of a residential homeParticipates and contributes to a treatment team of a residential home, including attending weekly team meetings and staffings, as requiredAttends training, seminars, and conferences as assigned by supervisory or administrative staffCompletes in a timely and accurate fashion all documentation required for the positionTreats with absolute confidence all information concerning agency residents and programsTransports clients (youth) and their families to mandated court hearings, school appointments, doctor appointments or other case related appointmentsPerform on call duties as assignedONE HOPE UNITED VALUESDo the Right Thing | We serve the best interest of our clients and staff.
