Develop and implement service plans to support family stabilization ?? Conduct weekly home visits and outreach activities ?? Facilitate healthy interpersonal relationships within families ?? Advocate for clients and assist with school adjustment and permanency planning ?? Attend and participate in multi-disciplinary staffings and court hearings ?? Maintain accurate and timely case records and progress notes ?? Collaborate with community agencies to ensure wraparound services ??
